Opsoclonus = irregular, spontaneous, multivectorial saccadic eye movements without an intersaccadic interval
(aka “saccadomania”, “dancing eyes”, and “ocular myoclonus”)

Opsoclonus are saccadic intrusions WITHOUT intersaccadic interval:
↔️ Ocular flutter = ONLY horizontal
↔️↕️🔁Opsoclonus = horizontal, vertical and/or torsional movements

Pathophysiology of OMS remains unknown, however is postulated to involve loss of inhibitory signals🚫🛑:
1️⃣ from Purkinje cells in the cerebellar vermis
2️⃣ or from omnipause cells which normally inhibit burst neurons responsible for generating saccades

OMS can be autoimmune, parainfectious, paraneoplastic, or idiopathic. Both humoral and cell-mediated immune mechanisms are suspected

OMS has been associated with a wide variety of viral or bacterial pathogens, including SARS-CoV-2 in #COVID19:

20-40% of OMS cases are paraneoplastic:
- 🧓 Small cell lung and breast cancers most common in adults
- 🧒🏽 Neuroblastoma most common in children
- 👩 Ovarian teratoma in women under age 30

- 🎗️ Early detection and treatment of underlying cancer if paraneoplastic
- 💉 IV corticosteroids, plasma exchange, IVIg, cyclophosphamide or rituximab
- 💊 Symptomatic improvement with clonazepam and topiramate
