عدم انتظام ضربات القلب هو اضطراب يحدث عندما تكون ضربات القلب إما غير منتظمة او سرعتها غير مناسبة ?

Causes might be because of various cardiac diseases or electrolyte imbalance
( هو انقباض عضلات القلب بشكل غير منتظم بسبب اما خلل بإنتاج النبضات الكهربائية من المصدر SA node او استجابة انسجة القلب للكهرباء او كلاهما)

Goal of therapy for

بالنسبة للهدف راح يعتمد على نوع الـarrhythmia
e.g: the goal of treating atrial fibrillation of atrial flutter are
-استعادة إيقاع الـ sinus rhythm
- منع مضاعفات الـthromboembolism
- منع حدوث further recurrences

المريض اللي عنده اعراض لPVC ومع عوامل خطرة للوفاة من عدم انتظام نبضات القلب (MI , LV dysfunction) حنقتصر العلاج المزمن على :

اذا كانت الأعراض شديدة institute synchronized DCC immediately to restore sinus rhythm
اذا كانت متوسطة او بدون اعراض treated initially with antiarrhythmic drugs
IV procainamide,amiodarone, or sotalol may be considered

For an acute episode most patients require& respond to DCC But often recurs rapidly after DCC, IV magnesium sulfate is the drug of choice for preventing the recurrence

❗Bradyarrythmias
▫Asymptomatic do not require therapeutic intervention.
▫A permanent ventricular pacemaker is the long-term therapy of choice for pt with significant symptoms.
Pt who remain symptomatic may benefit from adding an α-adrenergic stimulant such as
midodrine

▫If pt with Mobitz II or third degree AV block develop signs or symptoms of poor perfusion نعطيه atropine (0.5mg IV\3–5 min up to 3mg total dose)

Management of arrhythmia in pregnancy:
عند إحساس الحامل بالخفقان بدون أي تشخيص مسبق للأريثميا خط العلاج الأول سيكون (change life style )
لكن إذا الأعراض غير مستحملة فالخيار الأمثل هو beta blockers: e.g. metoprolol

كلاسB الدراسات لم تشير إلى أي خطورة على الجنين
