4/ The schema here is straightforward: one of the tubes that communicates with the outside and courses through the mediastinum must’ve ruptured.
5/ What you may not have known is that pneumomediastinum and pneumothorax often come together when the mechanism of action is alveolar distention and rupture. The CT below depicts this well.
