How to remember 🧠 drugs that ⤴️risk of skin hypersensitivity syndromes (SJS, DRESS, TENS)? 🧐
Back to med chem days! 👩🏻‍🔬
Let’s start with anticonvulsants 💊
💡Anticonvulsants with AROMATIC side chains = ⤴️ risk ⚠️
‼️⬆️Offenders: see 📸 below

Other drug structures ⚠️ that ⤴️ rates of skin reactions = sufanomides
🔑NOT all “sulfa” drugs confer equal risk - a true sulfanomide contains NH2-SO2 moiety or an NH-4 aryl-amine that are associated with ⬆️risk of allergic reaction (full topic will be on future🧵!)

Pharmacogenomic considerations 🧬
Common HLA associations w/ drug-induced hypersensitivity syndromes
🦠 Abacavir - HLA*B-5701
💊 Allopurinol - HLA*B-5801
🧠 Phenytoin/CBZ - HLA*B1502
+ many more!
Drug induced lupus like reaction is also rare, but can be severe and debilitating for pts. ☹️
A helpful acronym to remember drug induced lupus = CHIMP
✅C = chlorpromazine💊
✅H = hydralazine 💊
✅I = Isoniazid 💊
✅M = methyldopa 🤰
✅P = Procanimide▶️slow acetylators🔄
💡 Ending thoughts 💭
SEVERAL commonly used drugs can cause skin reactions (NSAIDs, antibiotics, APAP, etc) so always important 🔑 to keep on the differential. Also consider rare, idiosyncratic reactions such as...
⚠️ Warfarin induced skin necrosis 🤯
