"What do you see when you're walking HOME from church?"
She was an older woman with a right MCA syndrome, and I will never forget what she said when my attending asked her that question.

She was admitted on Saturday morning, and she was upset when she realized she would miss church. She only lived two blocks from the church, and walked there every Sunday. She hadn't missed a Sunday sermon in decades. 2/๐Ÿงต
I was a resident at the time, and the attending was chatting with her on rounds. He asked, "What do you see when you walk to church?"
And she answered, describing everything that would have been on the right side of the street if she was walking from home to church. 3/๐Ÿงต
"And what do you see when you're walking HOME from church?"
"The same thing, obviously," she said, but went on to describe entirely new landmarks... Everything from the OTHER side of the street.
She was having visuospatial hemineglect... of her memories. 4/๐Ÿงต
@prasad_sashank told me about another case in which a patient with neglect was asked to name U.S. cities, and only came up with ones on the East Coast.
East and west aren't actually left and right, but that's how they appear on maps. 5/๐Ÿงต
To which my response was... "Yeah, but that's just because you live in Boston. I know how you east coasters think about the rest of the country..." 6/๐Ÿงต
If I remember the story correctly, he went on to ask this patient to specifically name cities on the West Coast of the U.S.
The patient thought about it for a minute, and eventually came up a few:
- Naples
- Fort Meyers
- St. Petersburg.

His neglect of the LEFT ran so deep, the best he could do was think of the leftmost parts of the rightmost state!
That's the whole story. Just wanted to pass on another reminder of why the human brain never ceases to fascinate!
