The "Ghost Core" is important to be aware of in acute stroke triage decisions. Described here initially for CBV: pubmed.ncbi.nlm.nih.gov
2/ "Purple tissue" meaning the stuff that's purple on the Rapid Software (CBF<30%)
Traditionally conceptualized as "infarcted tissue" a better way to think of it is "tissue destined to infarct" (from @JimSiegler's @BrainWavesAudio podcast, open.spotify.com)

MR DWI signals reflect histologic changes, but in CTP what we call a perfusion "core" simply reflects that contrast is not arriving to these hypoperfused regions.
As time goes on, more of what is "destined to infarct" has occurred due to the poor perfusion.

But, particularly in the early time window, these hypoperfused areas may not yet have actually have undergone permanent ischemic damage.
In these early window cases, it can be easier to just use the traditional ASPECT score to determine candidacy for EVT.
