@Jwaitz @kdjhaveri @ASNKidney @Nephro_Sparks @NephRodby @danielwross4 1/5 Thanks for the reply Dr. Waitzman. Absolutely, there is scarcity of trained faculty as highlighted in a recent survey published in CJASN. Interestingly, critical care was in a similar situation a few years ago. Even now, not every intensivist is proficient in #POCUS though we

tend to assume otherwise. Having it as a core requirement motivates programs to take necessary steps. Change is not gonna happen overnight but 'friendly recommendations' won't help. I guess vague language in CC recommendations serves the purpose

of providing time for the programs to catch up.
It's also true that trainees are teaching faculty. But can we be sure that they are teaching it right (considering current heterogeneity in POCUS training among residency programs)? How do we know the IVC diameter documented by

them is accurate and not subject to cylinder effect? If they do any advanced stuff like VExUS without much experience just because its 'cool', how do we know they are doing it right? (For example, making false assumptions about hepatic vein when interpreting without EKG).

Overall, relying solely on trainees is a risky thing as opposed to programs investing time in faculty development - dedicated POCUS rotations must be arranged with non-nephrology #POCUS experts. @docanjuyadav did that recently with EM.

Basically I'm saying we must convince fellowship programs to invest time and resources into developing #POCUS curricula and its only possible when it is incorporated into formal guidelines.
In general, great job by the committee in addressing these key topics. Thanks.
