How do I want to approach chest pain? So common. So complex. Yet we need to start somewhere. Here comes my simple approach (for now).
CHEST PAIN
1) exclude life threatening causes (think 4+2+2)
clinicalproblemsolving.com
2) quality EXERTIONAL
prioritize cardiac ischemia
a) increased oxygen demand
b) decreased supply
3) quality PLEURITIC
any cause of pleural irritation (think: anatomic approach)
4) consider multiple processes if multiple qualities
5) abdomen and chest are neighbors (both might hang out in both places). Abdominal pain = cardiac/abdominal cause. Chest pain = cardiac/abdominal cause.
6) for further chest pain ddx -> anatomic approach
