The heart of the problem is assumed superiority. The other people they know assume they are superior to Midwesterners or Southerners.
They think they are more sophisticated. They think they are smarter because they talk faster (this is a big one, very important).

This next step is going to be difficult to handle for many: people who assume they are smarter than other people based on location are generally morons.
The average East or West coast urbanite is, in fact, not smarter in any way than the average Heartland resident.

The people in Ohio, Tennessee or Kansas are just as intelligent, but they are a legitimately different culture, where many things are handled differently.
People talk slower and more carefully not just because it's normal, but because expectations are higher as well.

Midwesterners are in general very polite, but that doesn't mean they are dumb suckers. No, we have expectations of politeness for how people are to behave, and we absolutely judge people when they are not polite.

This next part is where things get difficult to follow when someone moves to our area, doesn't get the culture, and considers himself to be superior.
We are (usually) much too polite to tell a coastal they are being a rude *sshole. Oh, if you understand us, there will be

multiple and unmistakable signs that someone is going over the edge, and quite a number of subtle shots may be taken, that still fall within the range of required surface politeness.
The "superior" coastal may not see any of that, they are blind & deaf to the culture.

If someone is used to a culture of loud exaggerations, and they move to a culture that is more subtle - they will miss much of what is happening and just call it being bland.

The coastal superior slumming in the Heartland fancies himself to be quite the c*cksman. But, in flyover country, for some bizarre reason, he just can't seem to get laid.

The truth is, of course, that even in any small Midwestern or Southern town, there will be women who enjoy a good time. Always have been.
