Pregnancy is a trying time. This is because anything can put both the mother and child at risk.
Due to this, the food and drug administration (FDA) classified drug use during pregnancy into 5 categories.
1. Category A
Drugs in this category are relatively safe.
Adequate and well controlled trials in pregnant women have found no risks of adverse effect on fetus.
E.g Pregnancy vitamins like B,C,D,E and folic acid.
2. Category B
Drugs in this category are safe still.
Animal studies show no evidence of risk, however there has been no controlled studies in pregnant women.
E.g Penicillins, cephalosporins, macrolides. These are antibiotics.
3. Category C
Drugs in this category are given when benefits outweigh the risks.
Animal studies has shown an adverse effect, however thereโ€™re no adequate studies in pregnant women.
E.g Antifungals and Anti malaria medications.
4. Category D
Drugs in this category are contraindicated in pregnancy.
Controlled studies in pregnant women have shown a potential risk to the fetus.
However, it can be given if the benefits outweigh the risks.
E.g Tetracycline, Streptomycin
5. Category X
Drugs in this category should never be given to a pregnant woman or women who wish to become pregnant.
It is absolutely contraindicated.
E.g Hormonal drugs, oral contraceptives, statins, valproate.
Drugs affect the fetus most especially during the first trimester.
This is because the organs are still developing and more susceptible to teratogenic effects.
However, there has been reports that these categories are over simplistic and not well defined, so the FDA introduced
..a new method of classification.
In this method, each drug or drug class is presented according to their;
- Level of exposure or need during pregnancy
- Summary of risks
- Clinical consideration (if it should be used or not)
- Studies and data available.
The safest drug to be used during pregnancy is no drug.
However, complications during pregnancy cannot be ignored for the sake of both mother and child.
All drugs should be prescribed by a doctor because he/she has accessed the risks and deemed it beneficial to you.
When getting drugs from a pharmacy, kindly state if youโ€™re pregnant or breastfeeding.
That would inform the pharmacist on drugs to dispense and the ones not to.
Always consult.
