Bone sustains millions of loading cycles over the course of a lifetime and rarely breaks without a major traumatic event.
It can sustain large deformations, sometimes at rapid rates.
Bones can adapt by simply adding more material and increasing mass.
We can take advantage of this for height growth!
Plastic deformation occurs in bones from incomplete fractures.
Incomplete fractures occur when a bone exceeds it's elastic limit but doesn't fracture.
If we look at the picture.
Entering the yellow strain zone will allow for permanent alterations in bone shape through plastic deformation.
Complete microfractures, occur within the cancellous bone.
These fractures undergo repair via endochondral ossification, where a cartilage or woven bone callus forms over the broken area.
For this growth mechanism to occur, the microfracture must be complete.
Complete microfractures will occur in trabecular bone.
Although the presence of a thin, dense layer of cortical bone surrounding it poses a challenge.
Otherwise, everyone would be growing taller by accident.
Incomplete microfractures happen when creating complete microfractures.
However, they do not heal through endochondral ossification.
Instead, they will gradually deform the bone over time. (plastic deformation)
When purposefully, creating complete microfractures in the trabecular.
Incomplete microfractures will also occur in the cortical.
To induce growth, we need to create compressive, tensile, and bending stresses on the bones.
The gradual process eventually fractures internal trabeculae, creating linear microcracks, diffuse microdamage, and microfractures.
This will exert stress on the cortical bone and enable entry into the yellow zone of plastic deformation.
Notably, the epiphysis lacks the thickness of cortical bone found in the diaphysis.
Thus, targeting more microfractures in the epiphysis becomes essential for effective height growth.
