To salt or not to salt? 🧂 That is the question in heart failure💔

Missed the SODIUM-HF trial discussed last week? Read on for a snapshot of this RCT of dietary sodium reduction in patients with heart failure!

What did I miss?
Studies have shown inconsistent results of the effect of dietary sodium restriction. Now, we finally have better RCT evidence.
SODIUM-HF = adults with NYHA class 2-3 & on optimum medical therapy randomised to usual care or a low sodium diet (<1500mg/day)

🔹 n=397 in low Na group, n=409 in usual care group
🔹 Primary outcome occurred in 15% of patients in low Na group & 17% of patients with usual care (HR 0.89, p=0.53)
🔹 Subgroup analysis found ⬆️quality of life & ⬆️one NYHA class in intervention group

Give me something clever to say
“Because the degree of dietary sodium reduction that would lead to a reduction in clinical events has not yet been defined, the pros and cons of dietary intervention of salt intake should be considered on an individual basis”

Chat consensus?
👍 Baseline Na consumption was generally low in both groups (~2g), is this representative of real life?
👎 No results on the use of diuretics in the patients
👉 Cardiologists & the @thelancet: 100mmol and 1500mg of Na is NOT the same

Final thoughts
✳️ Perhaps take the results with a grain of salt: could time spent on Na dietary counselling be better spent prescribing a flozin?
➡️ Up next - CLASSIC trial, on iv fluids in sepsis. See you on 9/10 August!
