Inositols are types of sugar alcohols that make up a component of the outer membrane of cells and thus help control what goes in and out of them.
Other than in food, MI is also produced in the human body from glucose and is present in all living cells as other forms such as phosphatidylinositol, D-chiro-inositol (DCI) and phytic acid (also known as Inositol hexaphosphate or IP6).
Our gut microbiome, with its bacterial phytases and phosphatases, is primarily responsible for digesting dietary IP6 in the body and for releasing myo-inositol and phosphate.
Though not classed as an essential nutrient because it is formed from glucose, it is very likely that a great majority of us are living with inositol deficiency and imbalance.
Firstly, caffeine (particularly coffee) increases the need for MI. MI also concentrates in tissues such as kidneys, brain, and blood cells, which are typically no longer consumed by us.
MI requirements rise with age, usage of antibiotics, consumption of sugar and refined carbohydrates, salt deficiency, insulin resistance, and both type I and type II diabetes.
Plus, the biosynthesis of myo-inositol requires NAD+ and magnesium. Thus, a deficiency in either may lead to a deficiency in MI.
Low MI levels have been seen in liver disease, stroke, certain thyroid diseases, lymphoma, and various other diseases. The presence of free-floating MI in the blood seems to deter the formation of fatty liver, and so it makes sense why MI deficiency can cause fatty liver.
Elevated blood sugar levels (via insulin) decrease the absorption and biosynthesis of MI and increase its degradation and excretion via urine. MI levels are higher in tissues that use large amounts of glucose, such as the brain, heart, and ovaries.
MI supplementation, especially in those with diabetes, has been shown to improve insulin sensitivity and is speculated to improve diabetes-related diseases such as eye and nerve damage.
Animal studies have shown that the administration of MI and IP6 independently has anticancer effects, and a combination of IP6 and MI has been shown to be significantly better at fighting certain cancers than either of the two acting alone.

Regardless of whether someone has PCOS or not, inositol imbalance has been postulated to lead to reduced fertility or pregnancy complications, thus, its administration may be useful in the prevention and treatment of several pregnancy-related pathologies.

MI seems to be vital for the proper running of the thyroid gland, and in one study, people with autoimmune thyroid disease were given MI and selenium, resulting in lower auto-immune thyroid antibodies, enhanced thyroid hormones, and better wellbeing.
